"Do" refers to a singular being, like "you". Since it's "windows" we know there are multiple of them. So "Are the windows cleaned once a month?", instead of "Do the windows cleaned once a month?". Since he "has" been painting, you can't use "was". So instead of "How long was he painting?", it's "How long has he been painting?". "Have" is used in the wrong tense here. Instead of "Who have been painting for fifteen years?", it should be "Who has been painting for fifteen years?".

"When he has been painting?" is fundamentally wrong. It translates into something like "Когда он был писал картины?". Instead, it's "How long has he been painting?" (you're curious about how long he's been at it, not when he started or finished). Again with the "is" being ignored and "didn't" coming out of nowhere. It's "Hot water is provided by the hotel 24 hours a day, isn't it?".
